I only found something how to bring notes to live, but thats not that what I want.
I already try something with the IAC-Bus, I dont know if it works...

On Mac you could use Jack, on Windows MidiYoke. Just set the right output in ableton and the right Input in PD and you'll receive midi data through the "ctlin" object.
